Clinical Pharmacist

Responsibilities

• Review medication orders for accuracy, appropriateness, and potential drug interactions
• Provide medication counseling and education to patients and their families
• Participate in medication reconciliation processes to ensure accurate medication histories
• Conduct medication therapy reviews to optimize drug therapy outcomes
• Collaborate with healthcare professionals to develop and implement evidence-based treatment plans
• Monitor patients' response to medication therapy and adjust treatment plans as necessary
• Document interventions and outcomes to support medication management processes
• Stay updated on advancements in pharmaceutical sciences and clinical pharmacy practice
  

Requirements

• PharmD degree from an accredited School of Pharmacy
• Valid license to practice pharmacy
• Proven experience as a Clinical Pharmacist
• Strong knowledge of pharmacotherapy principles and evidence-based medicine
• Excellent communication and interpersonal skills
• Detail-oriented mindset and strong analytical skills
• Ability to work independently and as part of a healthcare team
