Job Description

Roles and responsibilities

To provide pharmaceutical expertise and input to the care of patients and provide patients and clinical providers with therapeutic assessments and selected disease management, whilst working toward improving patient and clinical practice outcomes. The Clinical Pharmacist will ensure the delivery of International standards of care in accordance with established National and International practices, policies and procedures.

  • Contribute to developing pharmacy service standards and ensure they are consistently met. Act as a role model and deliver high clinical and operational performance standards by developing and supporting the Pharmacy Team.
  • Collaborates with nursing and allied health staff at all levels, interdisciplinary teams, executive officers and other stakeholders in the development, implementation, and evaluation of programs and services.
  • To market, promote and profile the service to patients, referrers and the general public and to undertake business development activities.
  • Ensures the reporting of all adverse events, incidents and near misses.
  • Contributes to investigations of complaints and incidents and draft responses as required in a timely manner.
  • Contributes to the monitoring of key performance indicators and developing action plans as required
  • Optimize patient care in defined wards/units by planning, managing, monitoring, advising and reviewing specialist pharmaceutical care programs for patients
  • Verify and monitor medication orders for drug interactions, duplication and omission therapy, allergies, adverse reactions, and recommend appropriate formulary alternatives as needed
  • Closely monitors medication error that may affect patient medication therapy and outcome.
  • Provides drug information and responds to drug related queries of all health care professionals.
  • Monitors adverse drug reactions and follow-up to recover ADR and to update documentation on patient profile.
  • Manages to present the scientific and clinical information of a new drug to be added in the formulary
  • Assess the status of the patient’s health problems and determine whether the prescribed medications are optimally meeting the patient’s needs and goals of care.
  • Evaluate the appropriateness and effectiveness of the patient’s medications
  • Recognize untreated health problems that could be improved or resolved with appropriate medication therapy.
  • -nterpret laboratory tests and other diagnostic tools to assess safety and effectiveness of drug therapy for individual patients and recommend changes to practice and policy in line with updated evidence based medicine.
  • Document therapeutic recommendations in the patient’s medical record and pharmacy intervention forms.
  • Follow the patient’s progress to determine the effects of the patient’s medications on his or her health
  • Consult with the patient’s physicians and other health care providers in selecting the medication therapy that best meets the patient’s needs and contributes effectively to the overall therapy goals.
  • Advise the patient on how to best take his or her medications.
  • Monitors restricted antibiotic use in the hospital.

Desired candidate profile

  • Pharmacology Knowledge

    • A deep understanding of how drugs work, their therapeutic uses, side effects, interactions, and contraindications.
    • Knowledge of the latest medications, drug formulations, and treatment protocols.
  • Patient-Centered Care

    • Ability to assess a patient’s needs, medication history, and individual factors (age, weight, allergies, liver/kidney function) to recommend the best treatment.
    • Expertise in providing counseling on the safe and effective use of medications, including proper dosage, timing, and potential side effects.
  • Drug Therapy Management

    • Monitoring and adjusting drug therapies to ensure effectiveness and minimize adverse effects.
    • Collaborating with doctors and other healthcare providers to select appropriate medications for patients, especially those with complex conditions or multiple health concerns.
  • Medication Safety

    • Identifying potential drug interactions, allergic reactions, and medication errors before they occur.
    • Ensuring proper storage and handling of medications to prevent contamination, degradation, or misuse.
  • Clinical Decision Making

    • Analyzing patient data (e.g., laboratory results, medical history, current medications) to make informed decisions about drug treatments.
    • Making adjustments to treatment plans based on patient progress or adverse drug reactions.
  • Patient Education

    • Teaching patients how to take their medications correctly, the importance of adherence, and managing side effects.
    • Explaining complex pharmaceutical concepts in a way that patients can understand and follow.
  • Interprofessional Collaboration

    • Working closely with physicians, nurses, dietitians, and other healthcare professionals to develop integrated care plans.
    • Participating in rounds and providing input on medication regimens for hospitalized patients.
  • Research and Continuing Education

    • Keeping up-to-date with the latest research, medications, and clinical guidelines to ensure best practices.
    • Contributing to research in the field of pharmacology, drug safety, or pharmaceutical care.
